To book an appointment, please call your nearest Whānau Health Centre.
You can also email a booking request with your full name and the date and time you would like the appointment on. We will then contact for confirmation.

To register with Te Korowai Hauora o Hauraki for all your medical services, please download our enrolment form here.
Once you have completed the form, please bring it to your nearest Whānau Health Centre along with your photo ID. Alternatively, if you prefer, you are welcome to collect an enrolment form directly from reception at one of our health centres or call us for assistance.
If you require a repeat prescription of your usual long term medication, please either contact your local Whānau Health Centre or, if you are registered with Manage My Health, you can request a repeat prescription through the Manage My Health app here.
When ordering your prescription through our health centres, please inform our reception staff or Practice Nurse if you require your prescription to be faxed to a pharmacy or if you will be collecting it in person.
All regular repeat prescriptions will be available to collect either at the pharmacy you have nominated or at the front desk of our Whānau Health Centres. Please note that prescriptions for new patients cannot be requested over the phone – you will need to make an appointment to see one of our Doctors (GPs) or Nurse Practitioners (NPs).
If it is more than 6 months since you have seen the Doctor or an NP, a repeat prescription may not be issued and you will be required to make an appointment to see the GP or NP in person.
If there are any issues with your request for a repeat prescription, one of our Practice Nurses will contact you.

We accept that not everyone may be able to do this, so payment plans can be set up to make it easier to cover these costs in advance or for whanau who already have an outstanding debt.
