Ohu HauoraWhānau Health Centres

Our Whānau Health Centres provide easy access to a range of health services such as GP or nurse appointments for the whole whānau.

All our services are accredited, accessible, affordable, of high quality and available to anyone in our wider Hauraki community.

We have five Whānau Health Centres located in Thames, Coromandel, Te Aroha, Paeroa and Whitianga. Contact details are listed here.

You can access any of our services as either a casual or a registered patient with no referral required.

  • To make an appointment as a casual patient, please call your closest health centre. Contact details can be found here.
  • To register with Te Korowai Hauora o Hauraki for all your medical services, please download our enrolment form here.. Once you have completed the form, please bring it to your nearest Te Korowai Whānau Health Centre along with your photo ID. Alternatively, if you prefer, you are welcome to collect an enrolment form directly from reception at one of our health centres or call us for assistance.

Ohu Kāinga Home Assist Services

Our Ohu Kāinga Home Assist service specialises in providing home-based care. Our range of services are based on the individual needs of Kaumātua and clients to support you to remain living in your own whare.

Together we aim to maintain your independence, your dignity and your quality of life.

Ohu Kāinga services are fully funded for eligible kaumātua and clients. Please discuss your eligibility and the referral process with your GP or Practice Nurse.

The Ohu Kāinga team of service coordinators and support workers will work alongside you to develop a Service Plan that will meet your individual needs. The team are dedicated, professional, experienced and care deeply for the whānau that they support.

Ohu Kāinga services are available across the Hauraki rohe in the following areas:

  • Whangamatā
  • Waihī
  • Paeroa
  • Hauraki Plains
  • Te Aroha

Tamariki Ora Child Wellness

Our Tamariki Ora/Well Child service is for children of all backgrounds aged 0 to 5 years and their whānau.

The service provides assistance and education to mothers and guardians about the care and wellbeing of their babies.

On-going assessment and assistance during these early years ensures our tamariki get the best possible start in life. Regular milestone checks conducted by our nursing team enable us to identify specific needs and enable us to provide access to additional services where required.

Tamariki milestones and growth are monitored with regular checks at:

  • 6 weeks
  • 3 months
  • 5 months
  • 9 -10 months
  • 15 months
  • 2 years
  • 3 years

Four year olds are given a B4 School check - hearing, vision and growth - ensuring their readiness for school.

Kaumātua OraElderly Wellness

Caring for our kaumātua is one of the most important services at Te Korowai Hauora o Hauraki.

We provide a range of services tailored to meet the unique needs of Kuia and Koroua in our rohe including Disability Information and Advisory Services (DIAS), specialised Kaumātua Programmes and Mirimiri / Rongoā Māori.

The Te Korowai Hauora o Hauraki Kaumātua programme is driven by kaumātua, for kaumātua and offers health forums which help to keep the mind active.

He oranga anō o roto i ngā mahi o tēnei kaupapa hei hikohiko i te hinengaro, hei hirihiri i te ngākau, hei korikori i te tinana i runga anō i te koa me te hari.

The Kaumātua programme is free and is open to all Kuia and Koroua in Hauraki rohe including Thames,Paeroa, Te Aroha, Coromandel and Whangamata.

The door is open to anyone over the age of 50, male or female, no matter where you come from, or your ethnic group.

Alongside our Kaumātua programmes, our Disability Information Advocacy Support (DIAS) service works to support our Kaumātua. The service aims to prevent isolation for kaumātua living with disability issues, helping them to live independently and safely in their homes and in the community.

HinengaroMental Health Services

Our Hinengaro (Mental Health) services are available free of charge to whānau of all ages throughout Hauraki.

For whānau in distress please contact our immediate response hotline services on 0508 111 555.

Our service model is underpinned by mātauranga Māori (natural knowledge) and systems and processes that support whānau to manage and achieve mana motuhake (autonomy).

Whānau and kaitono (referrer) receive matataki (triage) directly when reaching out to us through the free phone immediate response hotline 0508 111 555. Whānau who are with kaitono and making the call together, complete the process together.

Details of wānanga (meetings, appointments) are confirmed and invitations received in real time.

Whānau are able to refer themselves by contacting any Te Korowai Hauora o Hauraki Whānau Health Centre.

Ohu Oranga o Te Tangata Community

The Public Health Services at Te Korowai Hauora o Hauraki cover an extensive range of services for all members of our local rohe.

In addition to our specialist Kaumātua Services, we also provide programmes such as Whānau Ora, Iwi Health Promotion, Rongoā and the Quit 4 Good Stop Smoking Programme, all aimed at improving the overall health and wellbeing of our Hauraki community.

Whānau Ora

Whānau Ora Navigators working alongside whānau to empower them to identify and achieve their goals. Whānau Ora services integrate with other in-house and community services. Our Whānau Ora employment and education coordinator offers driver licence programmes.

Iwi Health Promotion

Our specialist kaimahi focus on health promotion with rangatahi (youth) in the community through a range of initiatives, services and programmes. Our programmes are holistic with key messages covering sexual health, alcohol and drug awareness, physical activity and nutrition education.

Rongoā

Our specialised rongoā practitioners offer traditional healing practices across the Hauraki rohe from our Whānau Health Centres in Thames, Coromandel and Paeroa and also as part of the Kaumātua Programmes.

Quit 4 Good

Stop smoking with support from our team of trained Quit 4 Good coaches. The Quit 4 Good programme is free and offered in small group or individual sessions over approximately six weeks.
